What is urology? Urology is the study and practice of diagnosing and treating the functions and organs of the urinary tract and genitourinary system. Urology (Your ah la gee) doctors are called urologists (see what is a urologist?) or urology specialists that have completed medical school and 5 years of residency specializing in the different specialties of urology. The organs that are involved in that practice of urology are: the kidney(s), adrenal gland(s), ureter(s), the bladder, the prostate, the urethra, the penis, and testes. The functions that are addressed in the practice of urology are: Production, transport, and removal of urine. Production, transport, and insemination of sperm and semen. Hormone production and distribution of the adrenal glands.What is urology? COMPUTER BLADDER EVALUATION

COMPUTER BLADDER EVALUATION (URODYNAMICS)This is a computerized bladder evaluation that is done in the office in both men and women.In men it is done for evaluation of prostate and bladder dysfunction due to medical conditions like an enlarged prostate, Parkinson's disease, multiple sclerosis, spinal cord injury and sometimes in the evaluation of recurrent urinary tract infections. In women, it is an invaluable tool in diagnosing and assessing the different types of urinary incontinence and to plan appropriate medical or surgical treatments. In addition, medical conditions like multiple sclerosis, Parkinson's disease and spinal cord injury which affect women can also lead to bladder dysfunction.Frequently Asked QuestionsQuestion: How is the test done? Answer: This test is done by inserting a small catheter into the bladder, a small catheter in the rectum and attaching electrode patches to the muscles around the anus. These are connected to a pressure transducer that is then connected to a multi-channeled computer. In the doctor's presence, normal saline is run through a pump into the bladder in a very controlled manner while the urologist supervises the test, asks questions and interprets the data on the screen. The patient may be asked questions such as when to they feel a cool sensation in their bladder and when they have an urge to urinate. Sometimes the test may include urinating into a special commode, which is attached to the urodynamics chair while the catheter is in the bladder. This gives additional information on bladder dysfunction.Question: How long does this test take? Answer: This test takes forty-five to sixty minutes to complete. This includes the set-up time, the insertion of catheters and the actual performance of the test.Question: What are the side effects of this test? Answer: There may be minor discomfort on insertion of the catheter into the bladder. Thereafter, there is no discomfort during the test.Question: What is the recovery time? Answer: Patients may experience some discomfort when they urinate the next few times for which they will be given medication from the office. Patients can resume full activity immediately after this test.Question: What are the risks of this test? Answer: In a very small number of people, this test may cause a small amount of blood in the urine and have a very low risk of infection. To prevent this, you are given an antibiotic tablet to take before the procedure.

Cystoscopy

An office procedure that requires only local anesthesia and typically takes less than 5 minutes. It is performed to evaluate problems of the urethra, prostate, and urinary bladder. A small flexible instrument scope called a cystoscope is inserted into the bladder through the urethra (the tube that carries urine from the bladder to the outside) so the physician may see these organs.Indications For A Cystoscopy: This test may be performed for men with suspected urethral stricture or narrowing of the urine channel, enlarged prostate, blood in the urine and diagnosis and monitoring of patients with bladder cancer. In women it is done for the evaluation of urine leakage, blood in the urine and diagnosis and monitoring of patients with bladder cancer. Additionally, stents that are inserted into the kidney during lithotripsy of kidney stones are removed in the office using a cystoscope.Risks Of The Procedure: Most patients experience some discomfort on urination for 24 hours after a cystoscopy that is significantly relieved by medications provided from the office (Pyridium). This medication is taken immediately after the procedure and eight hours later and turns the urine an orange color. There is a minor risk of blood in the urine, which typically clears within 24 hours after the procedure. A very small risk of infection is almost completely eliminated by taking an antibiotic tablet immediately after the procedure that is provided by the office.Frequently Asked QuestionsQuestion: Is it painful? Answer: There is minor discomfort during the procedure and most people are back to normal within 24 hours.Question? Are there any restrictions after the procedure? What is the Prostate? Where is it located?The prostate is a secondary sexual organ exclusive to males which secretes products such as a protein known as prostate-specific antigen (PSA). Though females do not have prostates, they do have similar organs which also produce this PSA protein. The prostate is typically the size of an apricot, and it is located beneath the bladder and surrounds the upper urethra. Its position in front of the rectum makes the prostate accessible to touch during rectal examination. The prostate functions in conjunction with another pair of secondary sexual organs known as seminal vesicles to produce the liquid portion of semen. Despite this inclusion in processes of intercourse, the role of the prostate in human fertility remains debatable. The position of the prostate deep within the pelvis makes surgical access difficult.Lap Davinci Prostatectomy, Laparoscopic Urologic Surgery, Vasectomy, Laser Prostate SurgeryWhat else should I know about prostate-specific antigen (PSA)?Elevated levels of PSA are common in ejaculate, though the level of PSA which leaks from the prostate into the bloodstream is typically low. The level of PSA in the bloodstream is indicative of the probability for developing prostate cancer, and higher levels are associated with higher probability. Specific conditions such as infections or use of certain medications can affect the level of PSA without increasing the probability of prostate cancer. As such, each patient must be evaluated individually in order to interpret the credibility of a PSA level. Though PSA levels can be indicative of cancer, they are not a sure warning sign; this is why an additional test called a biopsy is required to diagnose prostate cancer.Lap Davinci Prostatectomy, Laparoscopic Urologic Surgery, Vasectomy, Laser Prostate SurgeryWhat purpose does a biopsy serve?During a biopsy, sample cores are taken from the prostate to be fixed in formaldehyde and stained to identify sections which represent different regions of the prostate. These cores are then examined through microscopy to identify cancer, infection, inflammation, or other distinguishable abnormalities. A biopsy core is only a sample of a prostate, and as such results are not always a definite representation of the condition of the prostate. While a biopsy which reveals cancer can be considered a definite source of diagnosis, absence of cancer in a biopsy does not always indicate absence of cancer in the prostate. Biopsy results are also subject to the interpretation of the biopsy cores, and for this reason communication between Dr. Gholami and your pathologist can ensure an accurate and impartial interpretation. The pathologist will prepare a report for Dr. Gholami which will explain the nature of cores sampled in the biopsy. If cancer is detected this report will include the grade of the cancer as well as the proportion of the core involved. This report should also describe any abnormalities visible in the biopsy cores. Even biopsies which are negative for prostate cancer should note these abnormalities, as they could lead to future cancer diagnoses.Lap Davinci Prostatectomy, Laparoscopic Urologic Surgery, Vasectomy, Laser Prostate SurgeryWhy are the classifications of prostate cancer important?Prostate cancer is described by two separate classifications. The first classification, the grade of prostate cancer, pertains to the shape of cancer cells. Prostate cancer cells vary in shape when viewed under a microscope, and these shapes have been categorized and numbered by a Dr. Donald Gleason. The Gleason score of prostate cancer is the sum of two of the most common shapes of cancer cells. The least aggressive of these cells is labeled 2, while the most aggressive is labeled 10. The Gleason score of prostate cancer determines how aggressive the cancer appears to the pathologist, and it is important in allowing Dr. Gholami to pair his patient with the most effective therapy available. The second classification, or stage of prostate cancer, simply indicates the extent of growth of the cancer.

MICROWAVE TREATMENT FOR ENLARGED PROSTATE
In the office we perform microwave treatment for treating the symptoms of an enlarged prostate. This is a minimally invasive technique done by inserting a special catheter into the bladder and using computer controls heating the prostate in a controlled manner. This results in a significant improvement of symptoms and in most cases can eliminate the use of medications for treating an enlarged prostate.

X-rays are done in the office to monitor the progress of kidney stones that are small and are expected to pass spontaneously as well as after ESWL (Shock-wave Treatment for Kidney Stones). We also perform IVPs (dye test of kidney and bladder) in the office.

An office procedure performed for elective sterilization in men. The tubes (vas deferens) that carry sperm from the testicles to the penis are cut through one or two small incisions on the scrotum. It typically takes around 20 minutes and requires only local anesthesia.

URINALYSIS
This is done by an automated machine that chemically screens the urine for the presence of blood, protein or infection in the urine. This is followed by examination of a centrifuged specimen under the microscope for the presence of red (blood) cells, white blood cells (pus cells), crystals that can cause stones and the presence of casts that can indicate kidney disease.

Treatment options offered in the office include (1)application of 25% Podophyllin, a potent, toxic chemical. This requires several weekly visits to the office to be effective, and (2)surgical destruction of venereal warts. This is done in the office using local anesthesia with minimal discomfort.

PROSTATE SCREENING
This is done by a digital rectal examination (DRE), which is done by inserting a gloved, lubricated finger into the rectum and examining the prostate for size, consistency, contour and any abnormalities. The second component is a blood test - a PSA (prostate specific antigen). Though this is not a perfect blood test, it does aid the urologist in making decisions about whether one needs to have a prostate biopsy to rule out early-stage prostate cancer or whether your prostate is in good health and can be followed annually.

PROSTATE BIOPSY
This is an office-based procedure that is done to rule out prostate cancer. It is done by inserting an ultrasound probe into the rectum after local anesthesia has been instilled. The urologist carefully scans the prostate in multiple planes to look for any abnormalities and measurements of the prostate are taken. Next, using an ultra-high speed, spring-loaded device, six to twelve biopsies of the prostate are taken in a systematic manner. These are then sent to the pathologist for evaluation.Frequently Asked QuestionsQuestion: How Much Pain Will I Experience? Answer: There is mild discomfort as the ultrasound probe is inserted into the rectum. Thereafter the local anesthesia significantly helps in reducing any discomfort that may be felt from the biopsy.Question: How Long Does It Take? Answer: The entire procedure takes fifteen to thirty minutes.Question: What Are The Risks of the Procedure? Answer: The risks of the procedure are (1)a small amount of blood in the urine, which is typically self-limiting, (2)a small amount of blood in the stool, which is mild, (3)blood in the semen with sexual intercourse which is not harmful and fades in a few weeks, (4)a very small risk of infection which is prevented by antibiotics taken before the procedure, and (5)a very small possibility that an extremely small lesion in the prostate may be missed on prostate biopsy.Question: What Is My Recovery Time? Answer: We generally encourage patients to be driven home that day, as some people feel slightly light-headed after the biopsy. We suggest that you refrain from any vigorous activity for 24 to 48 hours after the procedure such as exercising, driving a lawnmower or off-road driving. If you are on blood thinners, your doctor will ask you to discontinue the blood thinners before the biopsy and indicate when it is safe for you to resume taking those medications.Question: When Will I Receive My Report? You will typically be given an appointment one week after the biopsy is done to discuss the pathology report with your doctor.
